Lost or Stolen Card
In the event of a lost or stolen card, immediately contact a customer service representative at US Bank 24-Hour Customer Service at: 1-800-344-5696.
After contacting US Bank to cancel the account, the cardholder must complete a Cardholder Maintenance Request form and obtain AM and PI approval in order to have a new card issued. Contact the PCA at 619-594-5622 or sdsurfpcard@sdsu.edu with any questions.
